电影讲述了报社记者、亚洲业余网络悬疑故事作家追查十年前发生在“谜镇”的几件知名鬼事的真相，亚洲根据匿名电话来到了所有传闻最早开始的地方，而十年过去后，这个地方因为闹鬼已经成了无人居住的鬼楼。随着真相慢慢揭开，他自己似乎也陷入了危险之中……《恐怖笔记》打破常规恐怖电影叙事模式，采用笔记类多线叙事风格，以笔记本为载体、笔记类章节式呈现，将“后视镜”、 “宾馆浴室”、“公交车”、“可视门禁”五个群像众多的繁复线索有机串联，重力打造出“笔记未完，恐怖不绝”这一极致恐怖体验，力求强化群像人物设定之间的纠缠和影响。
